Help at Home Service Opportunity


Dundee Health & Social Care Partnership is pleased to invite applications for the above Service which will be run initially as a ‘Test of Change’ for a period of 12 months.



The Remodelling Care for Older People Report (DIJB21-2017), presented to Dundee’s Integration Joint Board (IJB) in June 2017, and the Proposed Model of Care for Older People – Business Case, presented to the IJB in October 2017, highlights that services face the combined challenges of increased demand for care, an ageing population and pressures on funding.  In response to these challenges, Dundee Health & Social Care Partnership, the Third Sector and others are progressing improvement work to modernise pathways of care.  This includes breaking down the boundaries between hospital and non-hospital care and moving care into communities.
