Healthcare and Wellbeing
The Scottish Government’s 20:20 Vision of Health and Social Care and Support is that:
“By 2020 everyone is able to live longer healthier lives at home or in a homely setting”
The Public Bodies (Joint Working)(Scotland) Act 2014 (the Act) requires NHS Boards and Local Authorities to integrate planning for, and delivery of, certain adult health and social care services. The main purpose of integration is to use the available resources to improve the wellbeing of people who use health and social care services, in particular those whose needs are complex and who require support from both health and social care at the same time.
A Dundee Health and Social Care Partnership has been established to lead this process. The new Dundee Integrated Joint Board will oversee the delivery of integrated health and social care services for adults in Dundee.
The Dundee Health and Social Care Partnership consist of Dundee City Council, NHS Tayside, and partners from across Third Sector and independent providers of health and care services. These organisations and agencies are working closely together to implement formal joint working and planning arrangements, with the aim of providing better, more integrated, adult health and social care services.
The Partnership will promote transparent and inclusive partnership working. Features of this partnership will be positive relationships and clear accountability and governance arrangements. These, along with the Strategic and Commissioning Plan, will improve outcomes for the population of Dundee.
The Integrated Care Fund has been made available to health and social care partnerships to support the delivery of improved outcomes from health and social care integration and help drive the shift towards prevention and further strengthen the partnership approach to tackling inequalities.
Dundee Voluntary Action has several projects funded through the Health and Social Care Partnership:
- Capacity Building Fund- Community Cars and Home from Hospital Scheme
- Small Grants Fund
- Community Companions
- Technology Enabled Care
Dundee Voluntary Action Mental Health Engagement team, co-ordinates and promotes the active involvement of people who have experience of mental health conditions in planning, shaping and evaluating services in the statutory sector. In addition the team also support the involvement of voluntary sector mental health organisations in the planning, review, and delivery of mental health services in Dundee.
We also work in partnership with the Community Development and Engagement Team at Volunteer Dundee.
